Nationally and locally in Virginia, there is increasing recognition of the effectiveness of Community Health Workers (CHWs) as team members in the care of persons with diabetes. As trusted members of the communities they work with, CHWs are well suited to provide diabetes self-management programs and offer ongoing support for adults with diabetes in addressing social and health issues.
Organizations have increased efforts to engage CHWs in the delivery of diabetes support, but there is a need to expand awareness about how to successfully integrate CHWs into health care teams, how to maximize the impact of CHWs in diabetes self-management, how to finance their role, and how to strengthen their linkage role between clinical services and community resources.
