With funding and support from the Centers for Disease Control and Prevention (CDC) and the Virginia Department of Health, the Virginia Diabetes Council (VDC) is pleased to offer a funding opportunity designed to help make a positive change in the trajectory of diabetes in Virginia. The DP 18-1815 LivingWell grant is focused on new programs or program expansion into new areas/populations as well as program support in high burdened areas.

FY2022-2023 DP 18-1815 LivingWell Grant

The purpose of the DP 18-1815 LivingWell grant is to:
  • Improve access to and participation in ADA-recognized/ADCES-accredited DSMES programs in underserved areas
  • Increase engagement of pharmacist in the provision of medication management or DSMES for people with diabetes 
  • Implement strategies to increase enrollment in CDC-recognized lifestyle change programs 

Available funding for 1815

We estimate the total funding for both DSMES and NDPP grants to be $30,000. Average funding amount is $5,000 per applicant; however, organizations can request more or less funding depending upon their plan of work.

Funding Guidance

DPP:

With guidance from the CDC, the Virginia Diabetes Council has decided to implement a “per participant Program Delivery Cost” of $500. This $500 cost may include: formal Lifestyle Coach training, Lifestyle Coach salary, space rental, marketing and advertising to recruit and enroll participants, and program materials and supplies. 

DSMES:

  • Allowable costs:
    • Office supplies
    • Education materials (printing or purchasing)
    • Food for demonstrations
    • DAPS/EMR fees
    •  Transportation
    • Program support incentives*
  • Not allowable costs:
    • DSMES start-up costs or participant fees/co-pays
    • Promotional or recruiting ads/materials
    • Food (except for cooking demonstrations)Personnel time to provide DSMES directly to individuals  

*Program support incentives cannot exceed a monetary value of $20 per participant for the grant cycle.

Timeline: 

  • Funding Opportunity Announced: Tuesday, April 12, 2022
  • Application Due Date: Monday, May 16, 2022 5pm
  • Funding Decisions Announced: Friday, May 27, 2022
  • Grant Cycle: July 1, 2022 – June 30, 2023

  • has a pending submission to become a recognized/accredited DPP or DSMES supplier, 
  • interested in becoming a recognized/accredited DPP or DSMES supplier, or
  • a pharmacy currently providing medication management or DSMES for people with diabetes
