Virginia Diabetes Medical Management Plan
Purpose: This plan should be completed by the student’s personal diabetes health care team, including the parents/guardians. It should be reviewed with relevant school staff and copies should be kept in a place that can be accessed easily by the school nurse, trained diabetes personnel, and other authorized personnel.
The Virginia Diabetes Council Schools Committee endorses the American Diabetes Association’s new nationally standardized Diabetes Medical Management Plan (DMMP). This comprehensive form allows the healthcare provider to provide more individualized care for students with diabetes during the school day. The DMMP should be completed annually or whenever the student with diabetes care changes. The parent/guardian is responsible for providing the updated forms to the school to carry out the prescribed regimen.
