- Warren Memorial Hospital
- Healthy Living Center

- Riverside Diabetes Service
- Sentara Virginia Beach General Hospital Diabetes Center
- Blue Ridge Medical Center
- Health Wagon
- YMCA of DC
- Winchester Valley Health Diabetes Education Program
- Central Virginia Health Service
- Eastern Shore Rural Health
- YMCA and Central Virginia and Central Virginia Health District
- Virginia Tech Foundation’s Virginia Cooperative Extension
- Rappahannock YMCA
